During this six-week program, you will learn easy, practical ways that will make a real difference in living well with diabetes. The classes occur in a fun, engaging and safe environment that promotes the sharing of information and connecting with others.

Diabetes Empowerment Education Program™ (DEEP™)
Curriculum Description
The Diabetes Empowerment Education Program, also known as DEEP™, is a series of six 2-hour classes designed for those with pre-diabetes or diabetes to gain a better understanding of diabetes self-care. A family member or caregiver of the participant is also encouraged to attend so they can also learn about the disease and ways to provide support.
If you do not have a copy of your A1C, lab results, please contact your primary care physician as soon as possible so that they can provide a lab order for you.
Participants must attend all six sessions to receive a gift card.
Course descriptions for DEEP™
1. Session One: Beginning Session and Understanding the Human Body
• Group exercises to get familiar with how the class works, and form unity and trust
• Description of how the human body works, and its connection to diabetes
• Tips to manage and control diabetes with the goal of understanding your self-care goals and values.
2. Session Two: Understanding Diabetes and Its Risk Factors
• The definition, classification and symptoms of diabetes
• Factors that can increase your risk of diabetes
3. Session Three: Health Management through Meal Planning
• Get ideas and learn basic nutrition terms so that you can make the right decisions when selecting foods
• Learning about portion size
• How to read and understand food labels
4. Session Four: Checking Your Body and Diabetes Complications: Identification and Prevention
• How to diagnose diabetes, hypoglycemia, hyperglycemia, and ways to control them
• Diabetes self-monitoring, setting a self-plan and the benefits of the glucose meter
• The major problems of diabetes
• Learning about the different types of specialists that can help you manage your risk of diabetes
5. Session Five: Get Up and Move! Physical Activity and Diabetes and Living with Diabetes: Involving Family and Friends
• The health benefits of daily exercise, how to manage risk of diabetes, and/or to gain better control diabetes
• The emotional weight of diabetes, such as stress and depression
• How to involve family and friends in the self-care program
6. Session Six: Learning about Medications and Medical Care
• Treatments offered for the control of diabetes, high blood pressure, high cholesterol and fats
• Medications, devices, recommendations and side effects
• How to improve speaking with healthcare providers
• Self-care guides for persons with diabetes
